How the Health and Care Worker Visa Works

The Health and Care Worker Visa is part of the Skilled Worker route. It’s for qualified healthcare and social care professionals who have a job offer from the NHS, an NHS supplier, or a CQC-regulated care provider. Your employer must hold a Home Office sponsor licence and issue you a Certificate of Sponsorship before you can apply. That certificate is valid for three months, so timing matters.

The visa lasts up to five years and can be extended. After five years, you can apply for indefinite leave to remain. This would give you the permanent right to live and work in the UK without immigration restrictions. Holders and their dependents don’t pay the Immigration Health Surcharge. They also enjoy lower application fees than the standard Skilled Worker route.

Health and Care Worker Visa Eligibility

Restrictions now apply to overseas sponsorship for certain care worker roles. . Nurses, doctors, physiotherapists, pharmacists, and a range of other clinical professionals remain eligible. Limited in-country transitional arrangements last until 22nd July 2028.

Salary requirements vary. Most applicants must earn at least £25,000 per year or the going rate for their occupation code. Roles on NHS or Agenda for Change pay scales are assessed against the relevant pay band. Applicants must meet the applicable English language requirement under the Immigration Rules. If you want to bring your partner or children as dependants, their eligibility needs to be checked separately. The rules have become much stricter in recent years.

Eligibility depends on your occupation and your employer. Most new applications must be for graduate-level roles at RQF Level 6 or above. Qualifying roles include doctors, nurses, midwives, physiotherapists, occupational therapists, speech and language therapists, and a range of other clinical and health management professionals. You must also have a confirmed job offer from an NHS body, an NHS supplier or a CQC-regulated adult social care provider that holds a Home Office sponsor licence.

New overseas applications for care worker and senior care worker roles closed on 22nd July 2025. Limited transitional arrangements remain for workers already in the UK who were sponsored in those codes before that date. This allows in-country extensions with the same sponsor until 22nd July 2028, provided specific conditions are met. If you think you may qualify under the transitional provisions, you should get specific legal advice.

For most eligible healthcare roles, your partner and children under 18 can apply to join you or remain in the UK as dependants. They benefit from the same reduced visa fee and exemption from the Immigration Health Surcharge. Each dependant must submit their own separate application with evidence of your relationship.

Applications on this route can get fast-tracked processing. After you apply only, give your biometric information and send your documents, you can usually expect a decision in about three weeks. This is much quicker than the standard Skilled Worker Visa. If you need a faster decision, a priority service is available for an additional fee. If you apply from outside the UK, you might need to visit a Visa Application Centre.

You would need to apply to update your Health and Care Worker visa if you want to change your job and your new job is with a different employer. If your job changes to a different employer you will usually need a new Certificate of Sponsorship and may need to submit a fresh application before starting the new role.
